Describe based upon your community assessment, what is the most challenging health concern identified?

Clearly state the community health problem.

Identify three factors that cause/relate to the health concern or problem.

List three signs and symptoms (data) that support the identified factors related to the problem. Examples would include record accounts, census reports, and vital statistics.

Design a community-based strategy that can be planned and implemented to decrease or eliminate this community problem.
